Payment Requirements |

Payment is due at the time of enrollment, or up to the day before the class. Please note that payment is not accepted by the instructor on the day of class. We accept a variety of credit cards and both personal and business checks.

Course Location Change |

In the unlikely event that a course location must be changed, you will be notified by our office and rescheduled to attend the next available class. There are no refunds for cancellations or location changes.

If You Decide To Cancel Your Enrollment |

If you have registered for a class, and you cancel your enrollment 5 days or less prior to the class date, you will be charged a $35 rescheduling fee. This fee must be paid prior to attending the rescheduled class. There are no refunds for cancellations.

Returned Check Policy |

In the event a check is returned by the bank for insufficient funds, a return fee of $35 plus any bank service fees incurred by Center Point Resources, LLC will be charged to the client and the enrollment cancelled until payment has been satisfied.

State of Illinois Certificate Fees |

Center Point Resources LLC does not issue your FSSMC certificate. Only the State of Illinois can issue the certificate. Here's how it works:
After successfully passing the final state examination in class, the Illinois Department of Public Health (IDPH) will mail you an application for your certificate. There is a $35 fee, payable to the IDPH. Submit the application and a check for $35 to the IDPH in Springfield, Illinois, and you will receive your certificate in approximately 6 weeks. The certificate must be maintained at your place of business and be available for inspection.
